Rebecca gives Jack his birthday dance on his and the kids’ first birthday. Rebecca has a sudden unexplained breakdown watching the kids eat their birthday cake. She explains that she’s still carrying grief about Kyle. They visit Dr. K. He talks about his experience losing his first child and then raising his second.
Kate, Toby, and Rebecca throw a first birthday party for Jack. Kevin calls Nicky from outside an AA meeting. It’s Kevin’s one-year anniversary of sobriety. Randall and the family head to LA for Jack’s birthday party. Beth disapproves of him forcing Rebecca into the trial for Alzheimer’s. At the party, Kevin and Randall have another tense conversation about it (Kevin hasn’t learned what Randall did). Rebecca announces her intention to do Randall’s clinical trial. Kevin and Kate are confused as to why Rebecca suddenly changed her mind. Kate and Toby leave to do something in secret – putting Jack’s photo up at the NICU where he was born. Toby recalls the first moments after Jack was born when he comforted Kate. He apologizes for not being his best since then. He says he wants to adopt another baby. Kevin confronts Rebecca and quickly figures out that Randall forced her into the trial. He and Randall get heated but are interrupted by Madison suddenly arriving.
A man tames a tough horse. His daughter manages to calm her easily, though. She points out that they need to let him run, and that they need to give him a chance to be himself. The man is a doctor, and he goes into work to visit his patient, a pregnant Madison. She’s pregnant from her one night stand with Kevin and hasn’t told anyone. She hasn’t even told Kevin because she thinks he wouldn’t want the baby. The doctor suggests she ‘give him a chance to be himself.’ She goes to the party and tells Kevin she’s pregnant and she’s having the baby. Overwhelmed, Kevin leaves the room to think and runs back into Randall, and they pick their argument back up. They each say horrible things to each other about Jack – Kevin says Randall should have saved Jack if he were at the fire, and Randall says Kevin is a failure for not living up to Jack’s expectations. They each walk away angry. Kevin returns to Madison to tell her he’s ready to be a father. Madison reveals that she’s having twins.
Lucy’s pregnancy is late. Jack plays her his most popular song to cheer her up, which somehow triggers labor. An art curator named Haley rejects her friend Zach’s romantic advances because she’s dating ‘a movie star.’ She gets a call at work and rushes out. It’s revealed that she’s Jack’s sister and she’s going to visit her niece in the hospital. They have a baby girl and name her Hope.
Old Kevin and Old Randall have a tense reunion at Kevin’s house. Kevin’s twin son and daughter are there.
